1-FOR FIRST REGISTRATION :

  2. Intermediate mark sheet.
  3. All-year marks for the pharmacy diploma or degree
  4. High school transcript.
  5. Six passport-sized colored photos.
  6. Intermediate Certificate.
  7. For Diploma Pharmacy applicants only, a 500-hour Practical Training certificate from PCI-recognized Government Semi
  8. Government and Government Aided Hospitals is required.
  9. For candidates who have passed from outside of Uttar Pradesh, a current domicile certificate (issued within six months) is required.
  10. A college-issued provisional certificate proving passing the pharmacy examination or a diploma or degree in pharmacy

2- FOR TRANSFER CASES :

  1. Intermediate mark sheet.
  2. Mark sheet for all years of the pharmacy diploma or degree
